Background Check
To complete and the necessary background checks in order to be permitted to be a team official with the Pembroke minor hockey Association the following is required:
- Complete the TEAM OFFICIAL INFORMATION sheet which includes a request to the relevant police services for a criminal record and vulnerable sector checks.Bring the completed form along with the required identification (As stated in the application) to the Risk and Safety Co-ordinator: Jean-Pierre Quintal who can be contacted at 613 735-5777(w) 613 401 4777 (C) or jpq@nrtco.net. He will photocopy and verify your Identification and arrange to deliver the necessary documents to the relevant police force.If you reside in the Pembroke Police Service jurisdiction a cheque in the amount of 10$ to the Pembroke Police Service will be required.
- You will be advised (If residing in the City of Pembroke) when your Criminal record documentation is complete and to attend at the Police Station to pick it up. Once you have the documents, the must be remitted to the Risk and Safety Co-ordinator.
